## Narrative

Employee was changing the jack on a roof bolter operator canopy. While the jack and canopy assembly was lying on the mine floor, the sliding canopy extension fell to the mine floor striking the victim on the right little finger. This resulted in a fracture and laceration requiring surgical repair.
