## Narrative

The employee was pushing a roof bolt into the mine roof with his left arm when he felt a sharp pain in his left shoulder. The employee was able to continue working at his normal occupation, with no restrictions, while attending therapy sessions. His Dr. decided he was going to need surgery in order to make a full recovery. He began missing work on 4/30/2013.
