## Narrative

Employee was climbing onto machine. He had 3 points of contact (left foot on track step, right foot on final drive, right hand on cab rail). When stepping up and shifting his weight, he lost his grip and balance, falling backwards. His hard hat came off, and he hit & cut his head on the bucket lip. He had a 2"" cut closed by staples at a local hospital.
