## Narrative

Employee had removed 3 of 4 bolts supporting the top fan shroud with a grinder and was preparing for final bolt removal.  He placed his left hand on the top of the shroud and his right hand at the bottom.  His left hand slipped off the shroud causing it to drop 6 inches, lacerating his right index finger.  He received 5 stitches.
