## Narrative

A man working for our fuel supplier Radio Oil Inc. had just completed filling our PC400 excavator and was in the process of climbing down when he fell approximately 2.5 feet onto the ground.  He landed on his back.  At this time he felt pain in his lower back and requested an ambulance. Ambulance arrived and he left at approximately 8:00
