Narrative

Employee experienced a shift change in his hearing. Audio base line was set in 2000. He was tested on 05/062013 and a shift was discovered in both ears. He was re-tested on 05/14/13 and a shift remained in both ears. He will see an audiologist to verify whether or not he had other independent medical issues.
