Narrative

Injured was sliding a hydraulic unit from truck, the unit toppled to the left snatching the left arm and shoulder of the employee. Injured treated In-house for severe shoulder strain; pain reoccurred 5/23/2013 exam was performed and MRI & Cervical X-Rays ordered. Employee placed on restrictions, Rx given for pain and muscle relaxation.
