## Narrative

A slab of mine roof fell striking him on his left side knocking him to the ground. The roof rock measured at its maximum distance 15 foot long x 40 inches wide x 8 inches thick. The mine roof looked normal and was about 7' 6"" tall. The section of roof that fell out had a polished slickenside surface above it that was not visible after the area had been mined and roof bolted.
