## Narrative

EE was helping lift a rolled up piece of rubber belting measuring 36""x25' when he felt pain in his back on 5/16. EE was involved in shoveling & sweeping on 5/29 around the plant when he felt more pain in his back, more of a strain. EE was seen by a doctor on 5/28 & sent home on 5/29 to rest, ice & take medications for strained back after more pain. He will see specialist on 6/11.
