Narrative

Spading a check curtain to the rib line and was using a roof bolt plate as a hammer to pound the spad in the rib. He missed the spad and struck his left hand with the roof bolt plate. PPE was a factor, compliance rules/regs was a factor, mining equipment/systems was not a factor, miner proficiency was a factor.
