## Narrative

An area of rehab had been identified in the 4440 access.  The area was immediately posted with warning signs preventing access.  A bolter was brought in and was in the process of resupporting the back when a large basket of loose rapidly formed just in front of the cab of the bolter and settled on the boom of the bolter.
