On 12/25/2012 employee was moving a pump & experienced some pain & discomfort in his left shoulder. He reported this & went to medic on 12/26/2012. At this time - it was a ""Report Only"" -no da ys lost. On 5/30/2013, he did end up having surgery on his left shoulder- 5/30/2013 is the 1st shift missed.
