## Narrative

Employee was struck by a mill ball that became dislodged on his upper middle back.  The employee was given a full duty release from NNRH ER Dr. the day of the injury. At a follow up visit employee was prescribed physical therapy deeming this injury to become reportable as of 06/05/2013.
