Per notice received from MSHA, Employee has been diagnosed with a sufficient degree of pneumoconiosis. Therefore, this report is being submitted. We have not received any notifications of his fillings per worker's compensation carriers(s) & neither have received notice either. This claim is not associated with an accident. Date of accident is date of notification.
