## Narrative

The #2 hoist had to be shut down because a loose hydraulic fitting sprayed fluid on the motor of the hoist. We were installing a new pinion brake and a mechanic forgot to tighten all the fittings. When we attempted to check the new system, the now charged hydraulic line sprayed fluid out of the coupling.
