Employee was bolting on the South Mains section, in the number 4 entry. Once the employee drilled the hole; he pushed the bolt to the top but the glue had already set up. The operator rotated the bolt, to try to mix resin, but the bolt snapped off and the 8""x8"" plate fell and struck the operator in the right forearm. The laceration took three sutures to close.
