## Narrative

EE was attempting to put antifreeze in the ""Gen Set"". When he removed the cap from the tank, air pressure blew cap upward, lost my balance and fell striking the tank.  Cause of this incident was the lack of awareness and not releasing pressure on tank. Compliance with rules & miner proficiency a factor, PPE, mining/systems not a factor. Filed due to a part 50 audit.
