On July 12, 2013, after 4 days off, an employee returned to the mine and reported that he had injured his neck on July 7. He did not report the injury to anyone prior to the 12th. Employee was treated for a neck strain and held out of service by the medical facility until after a follow-up visit scheduled for July 18, 2013.
