Narrative

Injured was operating chain hoists being used to lift a conveyor transfer chute into place. The upper hook of the hoist came free and released chute to swing in a downward arc contacting the injured shoulder. Employee has all proper training, was wearing safety gear, and was working within his job duty and skill level. No regulations were fouled. No mining equipment involved
