On 6-4-13 employee reported discomfort -Left elbow. There was no incident or accident. He said it was repetitive movement/motion due to years of mining. On 7-18-13, he went to doctor-restrictions were issued, in which we can accommodate & is currently working light duty. This is an Occ. Illness
