## Narrative

Employee states he was dismounting his dozer a D11 R and slipped on the step beside the engine compartment and fell between the dozer and tracks. On 7/28/2013 employee was treated at Boone Memorial Hospital by Dr. and diagnosed with a lower lumbar and left shoulder strain and taken off work. This is now a lost time injury.
