Narrative

Employee was changing the drill bit and was using a sledge hammer to strike the bit to loosen it for removal. He felt pain in his groin area after swinging the sledge hammer. Employee has continued to work and did not have any lost time until 8/5/2013. Procedures were diagnostic until 8/5/2013. Surgery was done today, 8/5/2013, to repair hernia.
