Narrative

Employee complained of pain in his right shoulder. After an MRI was performed there was evidence of degenerative changes to the AC joint but no evidence of any damage relating to any specific incident. This report is being submitted as an occupational illness. He started missing work on 08/05/13 and works a weekend/holiday schedule.
