## Narrative

EE stated that he had pain in his rt. shoulder that he experienced while doing work as a beltman. He was evaluated and RTW. Several weeks later he again complained and an MRI was performed. The results suggest degenerative changes in the AC joint and a Buford Complex condition of the shoulder. He had surgery on 8/8/13 and began missing work that day. This is an OCC Illness Report.
