Narrative

Employee was lifting up on a pipe wrench that was approximately 2 feet below waist level when he felt a pop in his right shoulder, and experienced immediate pain that radiated down the back of his arm, past the triceps to the elbow. Employee did not seek medical attention until 8/20/2013 and was put on modified duty as of that date.
