Injured failed to stop a track locomotive at a equipment door across mainline track. He hit the door with the locomotive causing the door frame to fall and lacerate his right elbow. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was a factor, and miner proficiency was a factor.
