## Narrative

Injured was trying to dislodge a roof bolt that was stuck under a long wall flight chain. It came loose and struck him in his upper lip. PPE was not a factor, compliances rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor. Miner went back to our Illinois mine. He is an employee of Murray Energy.
