On 9/11/13, EE was conducting his round and cleaning around the centrifuges at the Crystal Plant when an 8"" stainless steel intake pipe came loose from a flange and struck the employee from behind on the back/neck/shoulders. On 9/15/13, he continued to feel pain in shoulders & returned to clinic for medical evaluation.
