## Narrative

Employee was changing vertical shearer pin, he laid a new pin on relay bar. When the ram pushed the relay bar snapped up in the air the pin laying on that relay bar struck employee in forehead cutting him. PPE was not a factor, compliance rules/regs was not a factor, mining equipment was not a factor, miner knowledge and proficiency was a factor.
