Narrative

Employee was using a pick hammer to pound on his drill chuck to free up stuck drill steel on his roof bolter when a piece of metal broke off striking him in the forehead. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was not a factor, miner proficiency was a factor.
