Narrative

On 9-17-13 employee was attempting to lift a belt roller to release a chain and felt a pain in his right groin area. On 10-3 he was sent by his doctor and diagnosed with a hernia that would require surgery to repair it. Employee continued to work until 10-9-13, on 10-10-13 he had surgery.
