Narrative

While adjusting a jaw crusher, employee was stepping down off of the side track when he slipped, falling to the ground hitting his tail bone on a rock. Employee was examined by a doctor on 9/27/13 and given a full release. Clinic follow up visit on 10/3/13 supported full release. Employee reported increasing pain, was taken to a doctor on 10/11/13, and placed on restricted duty.
