## Narrative

Employee was removing the 5th roller pin on the right hand track frame that was stuck. Employee had welded a D-ring to the pin. As employee used a forklift to try and remove the pin, the D-ring broke from the pin and the D-ring and cable flew back and hit employees left arm.
