Narrative

On 1/23/13 a chunk of material rolled off of a mining face and made contact with the shovel, causing the shovel operator to be jostled. He was evaluated by a physician on 1/23 and released back to full duty work. On 10/10/13 he was prescribed physical therapy. He completed a ""series"" on 10/21/13.
