Narrative

Several employees were attempting to unplug the #2 - 4th stage cleaner feed pump with air. The 8"" HDPE suction line came apart at one of the welds. An employee was standing near the line when molly slurry inside the pipe blasted out. Some material got into the employee's eyes.
