Narrative

On 11/01/2013, EE was working on changing the OK coper filter solenoid valve on #1 and #2. When he returned to the shop he noticed discomfort in shoulder. He went to the clinic to be evaluated and then returned to work. On 11/12/2013, employee was in follow up appointment for shoulder and given Physical Therapy for shoulder pain.
