Narrative

EE was given the task to install a new pump. When he was searching for a bucket to dispose of broken grout he ducked his head to go under 2 low hanging rubber hoses. He raised his head, it contacted a stem of a gate valve resulting in him jarring his neck. He evaluated by a Physician and taken off of work for his next scheduled shift. Returned on 12-13-13 full duty.
