Narrative

Employee was up on #2 ball mill removing man head. Employee was using 20lb sledge to hit a wrench that was on a nut on the man head. While swinging sledge employee missed the wrench causing him to twist un-normally and tense up causing a muscle strain in employees lower back left side.
