Narrative

On 12/10/2013 an employee was moving approximately a 30 lb motor coupling hub from the top of the work table to the bottom shelf of the table. When he stood up he felt pain in his lower back. He reported to the clinic where he was just observed, no treatment. On 12/24/2013 the employee was prescribed physical therapy.
