## Narrative

Employee was cutting a small notch out of a roll of belt to determine the carrying side of the belt, which was to be added into the existing belt-line.  He was using a Lutz, Model 82 utility knife to cut the notch.  As he made the last cut, the knife slipped and the blade struck his left forearm.  He sustained a 2"" laceration to his arm which took 7 stitches to close.
