Narrative

Employee was driving a stake to support concrete forms. The hammer glanced off from the stake and struck the employee on the arch of his left foot. Employee was transported to a clinic for evaluation. X-rays indicate a non-displaced fracture if the first metatarsal bone. Employee was placed in a walking immobilizer/support boot and released to return to work with restrictions.
