## Narrative

A forklift was being towed up the slope haulage road by a diesel scoop. The forklift had to be disconnected from the scoop. Another employee asked the IE to check to make sure the tow bar was disconnected. When the IE picked up the tow bar, two 1.5"" diameter clevis's attached at the tongue of the tow bar shifted, smashing his right middle finger against the tow bar.
