Narrative

On 12/12/2013 at or around 1:00 AM Employee was climbing up the step of the 4001 Haul truck, he fell forward striking his right knee on the bumper of the truck. Employee continued to work as the days passed his knee kept bothering him so he went to the Doctor on 1/9/2014 Doctor placed him on light duty, starting 1/13/2014.
