Narrative

Employee was scaling down a rock when another rock fell hitting the slate bar and pushed into his Right thigh causing a laceration. His body wasn't properly positioned. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
