Narrative

EE tripped & fell over a 20""x20"" concrete footing that he did not see. He didn't have any complaints of pain nor told anyone he was hurt. On 11/27/13, he informed management this could be work related & requested to see a doctor. On 1/23/14, an Independent Medical Examination determined the prevailing factor for his back pain was the fall he sustained on 11/14/13.
