Narrative

He is a newly trained miner roof bolter who was using an air drill in an outby location. He broke company policies by leaving suspended loads unprotected by removing the bottom drill steel and leaving the top three in the hole without being held. The steels fell out of the hole striking his left thumb. He was wearing metacarpal gloves.
