## Narrative

On 7/30/13 EE was working on a D-11 dozer. He was in the process of lowering the belly pan. The pan sprung sideways and hit the EE on top of the shoulder. Initial med treatment and x-rays showed a contusion. The EE continued to work for over 5 months and saw a dr on 1/13/14. MRI showed significant inflammation of AC joint. Surgery 1/30/14. Fraying not one time incident per dr.
