## Narrative

See Attachment The EE is a surface lead warehouse /yardman and has performed multiple tasks for the last 5.5 years. He turned in an incident involving his neck which was reported as an injury at another mine. EE complains of neck pain and has been diagnosed with tendonitis and arthritis AC joint in his shoulder. He was scheduled for surgery 2/14/14 for his shoulder.
