Crew of three were moving refuge chamber with a scoop, the chamber was hooked in the scoop bucket with the winch rope hooked onto a 3/8 inch chain which was on the top of the chamber in a V shape so the chamber could be lifted up. After the move the bucket was lowered and the winch rope broke striking the employee in the lower right arm with the rope and hook.
