## Narrative

IE was rotating a tire manually on a 854K rubber tired dozed while standing on the push arm block when he slipped and caught himself.  IE began to feel pain in his shoulder.  2-13-2014.  IE was seen at the orthopedic specialist where it was determined that there was a severe rotator cuff muscle tear that will require surgery to repair making this a lost time injury.
